Early Intervention and Prevention
Working with DSP, CYFS, DHW, DOJ, Service Providers, families, other care professionals to support individuals in the early phases of crisis to provide necessary supports to avoid placement breakdown.
Our services are designed to meet individuals where they are and take an individualized approach to care.

Placement/Housing Options Available
We are available to provide emergency placements/respite services, complex or customized placements, transitional placements, and will work with various departments to develop and offer longer term placement options and solutions.
Length of service will be determined on a case-by-case basis and through work and consultation with the various departments, Service Providers, and families.
We offer an emergency and immediate specialized behavioral service 24 hours a day, 7 days a week.
Transitional/Follow-Up Services
Working with the individual to return to their placement or transition to a new placement option if required. We will offer follow-up support services to assist the individual in their home to ensure success and sustainability.
We will utilize Daily Logs to track information, provide supporting documents upon transition, and continue to offer extended services upon return to home or program.
Our service is based on five pillars of care, much like the grip on a baseball when throwing a “Change Up.”
The “Change Up” is regularly used to deceive a hitter. An individual’s behavior at times does the same thing, where a bad day or incident begins to define the individual rather than looking at the event. Treating this as one pitch that deceived us and adjusting with the individual for a better outcome in the future. Our services will treat all individuals with Dignity and Respect, offer services that provide Safety and Security, while taking an Individualized Approach To Care.
